Become A New SportLife Distribution Customer

1. Customer signing this agreement agrees not to sell any products via any third-party-facilitated online retail or online auction website (including, but not limited to: eBay, Amazon.com, NexTag.com, PriceGrabber.com, Shopzilla.com, Best-Price.com, or any other online retail or online auction websites which sell products similar in character to products). Customer acknowledges and agrees that this prohibition is necessary to ensure safety and the integrity of the products. Notwithstanding the foregoing, customer shall be permitted to sell individual unit-volumes of its products on its own direct website. SportLife Distribution reserves the right to prohibit sales of the products on customer's own direct website, at any time, upon written notice to customer.
2. Claims arising from invoices must be made within seven working days after delivery. In addition to all other amounts payable by Customer hereunder, Customer agrees to pay to SportLife Distribution upon demand, any and all late fees incurred including interest, costs and expenses incurred in collecting overdue and past due invoices including any court costs any reasonable Attorneys' fees which SportLife may incur in enforcing the obligations of Customer hereunder.

As the credit card holder, I hereby authorize SportLife Distribution to charge my credit card account and I hereby authorize receipt of merchandise at the shipping address below.
